This agreement must be signed by every party to be bound. A claimant's selection of “submission agreement” in an intake form does not commence arbitration, create respondent consent, or authorize a charge.
Parties and dispute
This Submission Agreement is made on [date] between:

The parties submit the following existing dispute to final and binding arbitration: [specific description, contracts, dates, claims and known parties]. Claims outside that description require written consent or a jurisdiction ruling supported by another arbitration agreement.
Agreement to arbitrate
The parties agree that the dispute shall be administered by the Anrak Arbitration Center under one of the following, selected and initialled below, incorporated by reference and supplied with this Agreement:

Exactly one rules pack must be selected. A rules-pack selection inconsistent with the agreed seat must be corrected by a written agreement signed by every affected party before registration.

Claims, interim arrangements and limitation
Attached Schedule A lists each presently asserted claim, defense, counterclaim and requested relief. Signing does not admit liability, waive a limitation defense or confirm tolling except as expressly stated here: [agreed tolling provision or “none”]. Existing court or administrative matters and interim orders are listed in Schedule B.
Procedure choices

Disclosure, data and AI
Each party identifies controlling affiliates, counsel, known witnesses and third-party funders in Schedule C for conflict screening. Parties consent to secure electronic case administration and the data practices attached as Schedule D. Material AI use and arbitrator use follow the locked Rules and AI protocol.
Entire agreement and signatures
This document and schedules are the entire submission agreement. Amendments must be written and signed by all affected parties. An invalid term is severed to the extent lawful without defeating the core agreement to arbitrate, unless applicable law requires otherwise. Counterparts and legally valid electronic signatures are permitted.

Execution evidence must preserve signer authentication, document hash, consent text, timestamp and completed signed copy. The Center registers the case only after verifying that every required signature is complete.
